This policy explains how we collect, use, store, and disclose your personal information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs), applicable state and territory health records legislation, and the Notifiable Data Breaches Scheme.

1. About Us

Peridot Clinic provides telehealth-based clinical services to support people wishing to safely and gradually discontinue antidepressant medication. Our program includes digital assessments, medical oversight, and secure communication tools to ensure continuity of care.

2. What Information We Collect

We collect personal information such as your name, date of birth, contact details, Medicare number, and health information including medical history, medication details, and clinical notes. Sensitive information such as mental health or treatment history is collected with your consent or as required by law.

3. How We Collect Information

We collect information directly from you via online registration, telehealth consultations, forms, and communication. With your consent, we may also collect information from your GP, psychiatrist, or pharmacist.

4. Use and Disclosure of Information

Your information is used to deliver and manage care, coordinate with your healthcare providers, evaluate services, and meet legal obligations. We only disclose information with your consent or where required by law.

5. Data Security

Your personal information is securely stored and protected through restricted access. 

Information no longer required is securely destroyed or de-identified in line with the Archives Act 1983.
